Each participant receives a comprehensive assessment to identify risk factors, strengths, and immediate needs. Based on this assessment, a personalized case plan is developed that includes goals related to education, employment, mental health, and family stability. Case managers work closely with schools, law enforcement, probation, and community partners to coordinate services and ensure consistent support.
Progress is monitored regularly, with case managers providing ongoing encouragement, accountability, and crisis intervention as needed. The goal is to help youth disengage from negative influences, strengthen family relationships, and build the tools necessary for a successful, gang-free future.
- Evidence-based curriculum- The CGF program utilizes an evidence-based curriculum designed to prevent and intervene gang involvement and promote positive youth development through education, self-awareness, and prosocial skill-building. The curriculum is rooted in research-based prevention and intervention models that address the risk and protective factors associated with gang membership, delinquency, and violence. The evidence-based structure of the CGF curriculum ensures that interventions are measurable, data-driven, and outcome-focused—empowering youth to make positive choices, build resilience, and create pathways toward a gang-free lifestyle.
- Case Management- CGF provides individualized, strength-based support to youth and families impacted by or at risk of gang involvement. Through a trauma-informed and culturally responsive approach, CGF case managers build trusting relationships that promote personal growth, accountability, and positive decision-making.
- Aftercare Services- CGF aftercare services are designed to provide continued support and guidance for youth who have completed prevention or intervention services and are transitioning to the next steps in life. The goal of aftercare is to ensure long-term success by addressing the ongoing social, emotional, and practical needs of each participant.
